# Turn meeting notes into decisions and actions: scoring guide

Turn rough meeting notes into a clean list of what was decided and who will do what by when, with each item tied to the line in the notes it came from.

## How to use it

1. Run the template on your own material, or take an answer you already have.
2. For each criterion below, read the answer against the two lines and tick one: meets or does not meet. Quote what you saw.
3. A criterion marked "checked by you" is yours to mark. An AI cannot meet it for you.
4. Count the ticks for meets, out of 5. Fix the first criterion that is not met and run the guide again.

## The guide

### 1. Every item quotes its note

A plain rule you can check by eye.

- Meets: Every decision, action and question quotes the line of your notes it came from, word for word.
- Does not meet: An item has no source line, or the quote is not in your notes.
- Your mark: [ ] Meets [ ] Does not meet
- What you saw:

### 2. Every action has an owner and a date, or is flagged

A plain rule you can check by eye.

- Meets: Every action has an owner and a due date, or is marked for follow up.
- Does not meet: An action has a blank owner or date and is not marked for follow up.
- Your mark: [ ] Meets [ ] Does not meet
- What you saw:

### 3. Every open question says who answers

A plain rule you can check by eye.

- Meets: Every open question names who answers it.
- Does not meet: An open question has no name beside it.
- Your mark: [ ] Meets [ ] Does not meet
- What you saw:

### 4. Every action quotes its note

A plain rule you can check by eye.

- Meets: Every action in the final list quotes a line that is really in your notes.
- Does not meet: A final action has a missing or reworded quote.
- Your mark: [ ] Meets [ ] Does not meet
- What you saw:

### 5. Nothing was added that is not in the notes

Checked by a second AI prompt.

- Meets: Every decision, action, owner, date and question comes from your notes.
- Does not meet: The lists contain an owner, date or item that the notes never gave, or one that was changed.
- Your mark: [ ] Meets [ ] Does not meet
- What you saw:
