# Write an incident report: scoring guide

Turn your rough notes on something that went wrong into a factual, blame-free incident report, with the timeline kept apart from the likely cause, and with personal and health details flagged for removal.

## How to use it

1. Run the template on your own material, or take an answer you already have.
2. For each criterion below, read the answer against the two lines and tick one: meets or does not meet. Quote what you saw.
3. A criterion marked "checked by you" is yours to mark. An AI cannot meet it for you.
4. Count the ticks for meets, out of 11. Fix the first criterion that is not met and run the guide again.

## The guide

A plain rule you can check by eye.

- Meets: The final text includes the line Check this report with the people who were there and with your own policy before you file it.
- Does not meet: The line is missing, or it has been replaced by a claim that the report is complete or safe to file.
- Your mark: [ ] Meets [ ] Does not meet

### 7. The likely cause stays out of the timeline

- Does not meet: The Summary or the Timeline states a reason or a guess about why something happened.
- Your mark: [ ] Meets [ ] Does not meet
- What you saw:

### 8. No name, health detail or private detail is in the final text

Checked by a second AI prompt.

- Your mark: [ ] Meets [ ] Does not meet
- What you saw:

### 9. No wording left that blames a person

Checked by a second AI prompt.

- Meets: No line judges a person's effort, care, speed or character, or says a person caused the incident.
- Does not meet: A line still blames a person, even in softer words.
- Your mark: [ ] Meets [ ] Does not meet
- What you saw:

### 10. Every event in the timeline table is in the final report

Checked by a second AI prompt.

- Meets: Every event in the timeline table is in the final report, in the same order, with the same time.
- Does not meet: An event is missing, changed or out of order.
- Your mark: [ ] Meets [ ] Does not meet
- What you saw:

### 11. A person who was there reads it before it is filed

Checked by you.

- Meets: A person who was there read the report, checked the timeline, and checked it against your policy on what must be reported.
- Does not meet: The report is filed unread, or without a look at your own policy on what to report and by when, so you cannot tell whether it is ready to file.
- Your mark: [ ] Meets [ ] Does not meet
- What you saw:
