Pull key facts into a table, for customer service teams in insurance

Pull the facts you name out of a text into a table, one row for each item, copying every value as written, quoting where each came from, and marking every fact the text does not give as Not stated.

This is the version for customer service teams in insurance. The steps and checks are the same as in the general template; each prompt carries one added line about your setting, and the examples are from your kind of work.

Two renewals with a date and an amount each, all stated plainly, so nothing is missing and nothing needs fixing.

Step 1 Chat Prompt

Review the exact prompt, then paste it into your own AI. For later steps, use the chat chain in Other Formats and Files.

You are a careful reader who finds each thing a text is about.
The text is an insurance record, such as first notices of loss, a renewal list or a call summary, and the table will go into a claim log or a service tracker.

Your task is to list every item the text mentions, where one item is: each policy

The text:
Two renewals are due. Policy one renews on November 1 and the annual premium is 1,120 dollars. Policy two renews on November 15 and the annual premium is 860 dollars.

Rules:
- One row for each item, in the order the text gives them. Do not merge two items into one row and do not add an item the text does not mention.
- In "Item", use the name the text gives it. In "Passage (quote)", quote the words from the text that first name it, word for word, as short as you can. Do not reword.
- Use only the text. Do not pull out any facts yet.

Return one markdown table and nothing else:
| Item | Passage (quote) |

Before Step 2, check: Every passage is quoted word for word from the text.

What This Sample Should Produce

Two rows should keep policy one renewing November 1 at 1,120 dollars a year, and policy two renewing November 15 at 860 dollars a year. The missing-facts list should be empty.
